Track I: Chronic Condition Management
Session 1.1: Is European Policy fit for personal care for chronic diseases?
Chronic diseases consumes the lion's share of European health care budgets. Two thirds of elderly Europeans live with them. Personal health systems can save money, empower patients and support professionals, but current policies often act as barriers to their adoption. This sessin explore what needs to change and who should drive change.
Session 1.2: Are Personal Health solutions the answer to better chronic condition management?
Personal health solutions, like diseases, come in many shapes and forms. In the home, on the move, or in the care setting, personal health solutions must be adapted to the professionals, to the patients, and to their operating environment. This session presents experience and lessons and learned from several countries.
Session 2.1: Is monitoring the best prescription for independent living?
Home monitoring solutions are increasingly applied in new housing developments for the silver generation. However, they require substantial investment and raise ethical and other questions. The session features current models and examine how these questions have been addressed in practice.
Session 2.2: How Can Europe better serve the elusive older European?
The demographic time bomb is ticking: fewer workers support more pensioners and pensioners are getting older. At the same time, the experience of ageing is rarely the same. While many need support to live in autonomy, others stay healthy and active longer. The elder generation is diverse and multi-faceted. This session will shed light on this diversity and feature different approaches that enable independent living.
Track III: Health & Wellness
Session 3.1: Can personal health systems create healthier lives?
Our lifestyles are profoundly affecting our health. Spreading healthier living habits will make a key difference for the sustainability of future health budgets. Personal health systems can be powerful triggers and motivators. This session presents latest trends in public health policies and ideas on how personal health systems can contribute to a healthier Europe.

Session 3.2: Can the wisdom of crowds drive care personalization?
The internet, social networks and the sharing economy are generating new health information and knowledge at a pace and scope heretofore unimaginable. How can we make good sense and use of this information and advance our health potentials? What are the benefits? What are the risks?
Track IV: Innovations in Personal Health Systems and care delivery
Session 4.1: Measuring the impact of innovative services and tools
As we move from the age of projects to the age of deployment, regulators and payers are stepping up their demand for proof that personal health systems hold up to their promises. The session features lessons from the assessment of a large scale effort, with a panel of fellow evaluators to comment and critique.

Session 5.1: Leveraging healthcare budget and consumer spending (description)
Those who pay are not always those who reap the benefits. That is one of the dilemmas of social health systems when facing decisions whether to invest in personal health solutions. Patients are more and more asked to directly contribute. The session will features insights from a European Commission perspective and a panel of payers and patients.
